chronique fr\u00e9quente, qui repr\u00e9sente la <strong>premi\u00e8re cause d\u2019hospitalisation apr\u00e8s 65 ans<\/strong>. Pourtant, un rep\u00e9rage pr\u00e9coce et une prise en charge coordonn\u00e9e permettent de limiter les d\u00e9compensations, d\u2019am\u00e9liorer la qualit\u00e9 de vie des patients et de r\u00e9duire les hospitalisations \u00e9vitables.<\/p><p style=\"font-weight: 400;\">Dans cette dynamique, la CPTS du Voironnais d\u00e9ploie un parcours coordonn\u00e9 d\u00e9di\u00e9 \u00e0 l\u2019insuffisance cardiaque afin de <strong>renforcer la coop\u00e9ration entre les professionnels de sant\u00e9 du territoire<\/strong>\u00a0: m\u00e9decins g\u00e9n\u00e9ralistes, cardiologues, infirmiers, pharmaciens, \u00e9tablissements de sant\u00e9 et autres acteurs impliqu\u00e9s dans le suivi des patients.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div Inter-CPTS. Cette approche consiste \u00e0 identifier de mani\u00e8re proactive les patients \u00e0 risque afin d&rsquo;intervenir avant la survenue d&rsquo;une d\u00e9compensation.<\/p><p style=\"font-weight: 400;\">En s&rsquo;appuyant sur la coop\u00e9ration entre les professionnels du territoire et le partage d&rsquo;exp\u00e9riences et de donn\u00e9es, cette d\u00e9marche permet d&rsquo;am\u00e9liorer le rep\u00e9rage, la pr\u00e9vention et la prise en charge des maladies chroniques.<\/p><p style=\"font-weight: 400;\">Notre \u00e9quipe pluriprofessionnelle en charge de cette d\u00e9marche se compose d&rsquo;une m\u00e9decin coordonnateur, d&rsquo;une cardiologue, de trois infirmi\u00e8res, d&rsquo;une pharmacienne, de deux di\u00e9t\u00e9ticiennes, d&rsquo;une pharmacienne biologiste et
